Patient Rights & Responsibilities
The Staff at Stony Point Surgery Center pledge to do their personal best to provide our patients and their families with the highest quality of care and excellent service. In addition each patient has the right to the following:
- To receive considerate, respectful, and safe care by competent personnel with appropriate credentials.
- To be provided to the extent known, complete information regarding diagnosis, treatment and prognosis, and the possible risks and side effects associated with treatment.
- To be fully informed of the scope of services available in the facility, provisions for emergency care, related fees for services rendered and payment policies.
- To make informed decisions regarding care and treatment and to receive information necessary to make those decisions.
- To refuse care and treatment and to be informed of the medical consequences of such a refusal.
- To refuse to participate in experimental research.
- To have an Advance Directive, such as a living will, as required by state or Federal law and regulation.
- To personal privacy and confidentiality of medical records.
- Access to information contained in his/her medical record within a reasonable time when requested.
- To marketing and advertising by the Center that is not misleading.
- To appropriate information regarding malpractice coverage.
- To change primary or specialty physicians or dentists if desired, and other qualified physicians or dentists are available.
All patients and their families are considered to be participating members of the health care team and are expected to take an active role in their care and treatment to the extent they are able. Each patient has specific rights to which they are entitled and the following responsibilities:
- To provide complete information about his/her health including past illnesses, hospitalizations, surgeries, and use of medications.
- To be considerate of other patients, the staff, and the property of the Center, and to assist in the control of noise, smoking and other distractions.
- To report whether or not he/she clearly understands the planned course of treatment and what is expected of them and to ask questions when clarification is needed.
- For keeping appointments and, when unable to do so for any reason, for notifying the Center and the physician.
- For following instructions regarding medications and pre or post procedure instructions and for making healthcare providers aware if they do not understand the proposed treatment or any expectations of them.
- For promptly fulfilling his or her financial obligations to the Center.
- To provide the Center with accurate and complete information necessary for billing and processing claims.
- To promptly report any complication to their physician in a timely manner.
- To voice complaints and grievances without discrimination or reprisal and have those complaints and grievances addressed in a timely manner.
